The Medicaid Service Coordinators assist with developing and implementing an Individualized Service Plan, linking and referring to services, monitoring and follow-up service delivery and advocating for health, safety and well-being. We provide individuals with choice in their service delivery, including options for living, working or attending a day program. We also have successfully assisted with the exploration and identification of life goals, referral to appropriate programs and services, and the monitoring of satisfaction with services. A strong emphasis is placed on providing assistance and advocacy that include a wide variety of areas such as medical concerns, housing needs, coordination of treatment and transportation and guidance in accessing community services.
Individuals receiving services enjoy the benefit of having a team that supports their residential setting as well as their day services providing a coordination of care. The Medicaid Service Coordinator is often the major support to the person and works to ensure that all needs are met to enable individuals to successfully live independently in the community.
